Membership Application
To become a member of Entrust Federal Credit Union, you must establish a Savings Account by filling out a Membership Application and Member Identification Form.
Member Identity Information: Complete all information available to you at this time
Government Issued Identification: Complete with SSN or indicate other form of ID if you do not have SSN
Document Verification: Indicate and fill in which form of ID you are providing a copy of with application
Notary Information: Form must be notarized if not completed by an Entrust employee. This requirement ensures that the information on the form was completed by the person signing the form.
Staff Completion Documentation: Leave blank - to be complete by Entrust staff on receipt.
Once you have filled out and signed the Membership Application, please mail it to Entrust Federal Credit Union with a copy of primary and joint members' driver's licenses as well as a check for $25 that will be deposited into the Savings Account to establish your membership.
